We came across this error last week, the problem occured when any user other the impersonated user (from the plugin registration step) tried to fire of the plugin.
After exhaustive investigation it was discovered that the users were not added to the PrivUserGroup, obviously strange as thats CRM's Job!
One of those quirky things i thought might save you guys the hassle of looking into.
